What is the telematics infrastructure and why is it important for care?

The digitalisation of care requires secure, standardised processes - this is where the Telematics infrastructure (TI) comes into play. It connects all players in the healthcare sector - including care facilities, doctors, pharmacies and hospitals - via a protected, nationwide network.

Through the KIM service (communication in medicine) and the electronic patient file (ePA) ensures the secure exchange of sensitive health data. Only authorised persons are granted access.

A practical example from everyday care work: the electronic proof of performance (eLN). Since 1 December 2024, outpatient care facilities have been using the KIM service to record services digitally, have them confirmed by the patient and transmit them in a legally valid manner. This enables data protection-compliant billing with health insurance companies - efficiently and securely.

1What is the telematics infrastructure for care?

The telematics infrastructure (TI) is the platform for healthcare applications in Germany. The TI is a closed and secure network via which service providers can communicate with each other and, if required, with patients and health and care insurers and share documents.

2Why do care facilities need to be connected to the telematics infrastructure?

The TI is intended to connect all stakeholders in the healthcare system and thereby simplify communication. The interaction should be secure and data-protected compared to previous communication channels such as unencrypted email services or standard messaging apps. For the care sector, for example, this means that prescriptions can be sent more quickly to the responsible outpatient or inpatient care facility and medication orders can be placed faster and more efficiently. Sending or faxing prescriptions, findings or laboratory reports is no longer necessary and can instead be sent via KIM. Communication between medical providers and nursing staff can therefore be handled easily via TI applications. An additional advantage: the nursing staff gain a better overview of their patients' health situation.

3Who operates the telematics infrastructure?

The telematics infrastructure is operated by gematik GmbH on behalf of the federal government. gematik defines technical standards, certifies providers and ensures that all participants can communicate in a data protection-compliant and interoperable manner.

4When will the telematics infrastructure be mandatory for care facilities?

From July 2025, connection to the TI will be mandatory for outpatient and inpatient care facilities! From this date, certain digital applications such as the electronic service record (eLN) must be used.

5What applications of the telematics infrastructure are there for care?

The most important functions of the TI include the electronic patient file (ePA), the electronic prescription (ePrescription), the electronic medication plan (eMP) and communication in medicine (KIM).

6What advantages does the TI offer carers?

The telematics infrastructure provides nursing staff with fast, secure and digital access to relevant health information such as medication plans, emergency data or electronic prescriptions. This saves time in day-to-day care, reduces sources of error and improves communication with doctors, pharmacies and other care providers. Standardised processes and digital documentation relieve the burden on nursing staff - leaving more time for the actual care of people.

7What technology is needed for the TI?

A number of technical components are required to connect a care facility to the telematics infrastructure (TI). Firstly, a so-called connector is required, which acts as a secure interface between the facility and the TI. This is used for encrypted access to the network. In addition, an institution ID card (SMC-B card) is required, which uniquely identifies the care facility in the system. Another key element is a TI-capable software solution, such as TIPaaS from C&S, which enables simple and flexible integration of all necessary TI services.
